Letter of Authorisation

If you wish to make an appointment to discuss matters on behalf of a friend or family member, you will need them to authorise this in advance (the appointment cannot go ahead without it). The form will download to your device. Don't forget to save it once you have completed it and have it ready to be emailed to Paul at your first appointment.
